Introduction to Your On-Line 401(k) Plan Resource Center The purpose of this booklet is to provide a step-by-step guide for our plan participants to utilize our "24/7" 401(k) investment center. Our goal is to provide participants easy access to their account information with the click of a mouse. This booklet will walk you through the entire process of accessing your account, as well as making various changes in your personal information and your plan investment choices. If you have any questions after completing your review of this Guide, please contact Qualified Plan Administrators, Inc. at one of the numbers provided. Effective January 1, 2005, all trades placed before 3:00 pm will be sent to the broker for execution the same day. All trades places after 3:00 pm will be sent the next business day. All participants, who make changes to their accounts, will receive a written confirmation form within 5 business days. Qualified Plan Administrators, Inc., nor the Plan Sponsor, will be responsible for trades incorrectly placed by the participant. Please be sure you have input the correct information prior to submitting a change. If, after inputting changes, you think you may have done something incorrectly, please contact a QPA representative immediately.
