Beneficiaries

This screen will show you the name of the person or entity you have indicated as your beneficiary.  If no beneficiary information is on file, you will see a message indicating this and will be given the opportunity to complete an on-line beneficiary form.

If you want to add a beneficiary, or make changes to your current beneficiary, simply complete the form and click the Submit or Save Changes button.

You should then access your plan's website forms, and complete a Beneficiary Form that must be submitted to your Employer.  Remember, if you name someone other than your spouse as beneficiary, the form must be signed by the spouse, then notarized, to make it acceptable.
